Proposed DSM-5 will result in almost 50% fewer persons being diagnosed with ASD
Licensed Psychologist Johnny Matson, PhD, a Distinguished Research Master and Director of Clinical Training and Professor at Louisiana State University, recently completed a study evaluating the prevalence rates of autism and autism symptomatology in toddlers using DSM-IV vs DSM-5 criteria. (Matson JL, Kozlowski AM, Hattier MA, Horovitz M, Sipes M, Dev Neurorehabil. 2012;15(3):185-90).
2,721 toddlers at risk for a developmental disability participated in the study.
Two thousand seven hundred and twenty-one toddlers at risk for a developmental disability participated in the study. DSM-IV and DSM-5 criteria were applied and overall prevalence using each set of criteria was established. Groups were also compared on BISCUIT-Part 1 scores to determine if groups differed on autism symptomatology. Results: DSM-5 resulted in 47.79% fewer toddlers being diagnosed with ASD compared to those on the DSM-IV. Toddlers diagnosed according to DSM-5 exhibited greater levels of autism symptomatology than those diagnosed with DSM-IV, but the latter group still exhibited significant levels of autism symptomatology.

Conclusion: The proposed DSM-5 will result in far fewer persons being diagnosed with ASD. These results replicate findings from two previous studies (McPartland JC, Reichow B, Volkmar FR), with older children/adolescents and adults. As a result of these new criteria, far fewer people will qualify for needed autism services.

Bill Limiting the DSM5 Passes Illinois Legislature

A bill that would limit the impact of the Diagnostic and Statistical Manual, Fifth Edition (DSM5) passed both Houses of the Illinois State Legislature and is awaiting the signature of Governor Pat Quinn.
This bill provides a firm statement of no confidence by the State of Illinois in the American Psychiatric Association’s bungling of the DSM5.
Senate Bill S679 would allow people with a current diagnosis under the DSM4 criteria to keep their autism diagnosis regardless of what happens with the DSM5. This is an important first step to protecting people with autism from the reckless changes in diagnostic criteria proposed for the DSM5. This bill provides a firm statement of no confidence by the State of Illinois in the American Psychiatric Association’s bungling of the DSM5.
The DSM is the document written by the American Psychiatric Association (APA) that defines mental health disorders and the criteria used to diagnose them. A new definition of autism is proposed for the DSM5 which would result in a huge reduction in the number of people with an autism diagnosis.

Objective: This study evaluated the potential impact of proposed DSM-5 diagnostic criteria for autism spectrum disorder (ASD).*
Conclusions: Proposed DSM-5 criteria could substantially alter the composition of the autism spectrum.*
"DSM-IV Asperger's has four problems listed in its 'A' criterion and Only Two Of Four are required. So DSM 5 requires a 100% hit rate for A criteria problems; while DSM IV requires only 50%. The rates of Asperger's should drop precipitously under DSM 5 rules."

Individuals clinically diagnosed with autistic disorder were more likely to pass the DSM-5 diagnostic threshold than those with Asperger's disorder or PDD-NOS (75.8%, 25.0%, and 28.3%, respectively)**
According to the proposed criteria, cognitively able individuals and those with ASDs other than autistic disorder would be less likely to receive a diagnosis on the autism spectrum. The proposed change to a spectrum approximates a diagnostic construct closer to classic autism.**
- Research Derailed:
Potential for historical data to become irrelevant, as well as negative impact to consistency of meaningful data in future;
Negative impact to phenotyping of ASD, particularly where environmental triggers are concerned;
Lack of urgency to conduct research resulting from drop in prevalence;
Fewer research dollars assigned to prevent and treat ASDs;
Less research to translate into treatment.
